When booking your first appointment and any recurring appointment,

PLEASE CHECK YOUR EMAIL for our digital health intake form to be filled out PRIOR to your arrival. This also applies to existing clients to make sure their intake forms are up-to-date before every appointment. 

Why?

• It gives us a chance to review any issues of concern and to make sure there are no contraindications to receiving a massage. 
• No one likes their session cut short. If it is not filled out/updated when you arrive, we may not have time to give you the whole session you booked/paid for. 
• Existing clients: yes, we know we just saw you recently, but has anything changed or happened in the meantime that needs updated? 

⭐️ If you'd like to book a session for someone other than yourself, please do not book it under your name and email. This makes it complicated for the us and the booking system and takes time away from their session to clear things up. We need to have the email address and phone number of the person receiving the massage so we can send them a health intake form of their own.
